15 Hours per week - Permanent
We are looking for a dynamic person to work within a multi-disciplinary team in a community setting, providing support, care and treatment to individuals in their own homes. This service is provided between the hours of 07.00hrs and 22.00hrs over 7 days per week. There is a requirement to work shifts and weekends.
You will interact with all members of nursing, therapy, medical and other allied professionals related to the community team. You will be required to communicate with team members, patients and carers.
